Esther Mensah needs assistance to reposition after a recent illness. A carer has noticed a change over her sacrum. Assess pressure injury risks and the skin change, agree prevention and review, and complete the pressure-risk record. The patient answers personal questions; the examiner supplies requested examination findings. RN Alex Morgan is available.
What this station tests
- Suspected deep tissue pressure injury in darker skin may appear as local darkening with warmth, firmness and pain; colour alone cannot determine a stage.
- Offload suspected pressure damage and avoid massage or ring cushions.
- Repositioning, support surfaces, moisture and nutrition need an individual plan.
