Hari Thapa reports two recent near falls when rising from bed. He is at home for a community nursing assessment. Identify modifiable fall risks, agree an individual plan and complete the falls record without inventing a numeric score. The patient answers personal questions; the examiner supplies requested examination findings. Dr Taylor Singh is reachable by phone.
What this station tests
- Stop supported standing measurements if the patient becomes symptomatic.
- A useful falls plan links identified risks to practical changes and the person’s goals.
