The task

You are the RN on a hospital ward. Samuel Lee, aged 76, is preparing for discharge after an uncomplicated admission. His signed plan includes a GP appointment in three days and a reconciled unchanged medicine list. Make the discussion accessible, explain the discharge plan and check understanding. Complete the supplied practice record. You and the patient are in the room; assistance can be called. You have eight minutes; describe and select physical actions in this online exercise.

What this station tests

  • Samuel’s polite agreement may mask missed hearing access.
  • Check the aids in his drawer and speak face to face in a quiet setting.
  • Explain completed antibiotics, unchanged medicines and the 2 November appointment.
  • Teach-back must include infection return symptoms as well as routine follow-up.
