The task

Tess Martin has received wound care and is awaiting the next ED review. Take a focused update from her and hand over to receiving RN Casey. The ED clinician reachable by phone can be called if needed. Complete the handover record.

• Physical wound treatment does not resolve ongoing self-harm risk.
• Ask about ingestion and accessible means directly.
• Observation responsibility needs explicit acceptance during staff changes.
