You are a general practitioner. Mr Anil Desai, aged 54, has noticed blood when he opens his bowels. He is alone. Take a focused history. Then tell him what you think the possible causes are and what needs to happen next, and answer his questions. There is no need to examine him. The examiner will not take part in the conversation.
What this station tests
- Rectal bleeding with a change in bowel habit, weight loss or a family history is not haemorrhoids until proven otherwise.
- Ask about family history and screening participation; both change the urgency and the conversation.
- A digital rectal examination and a full blood count are part of the first visit.
- Acknowledge embarrassment and practical barriers, and offer help to overcome them.
- Safety-net for heavy bleeding, severe pain or fainting.
