You are a general practitioner. Mrs Priya Raman, aged 56, has come to your clinic because of chest discomfort. She is alone. Take a focused history. Then tell her what you think may be happening, what needs to happen next and why, and what she should do if things change. There is no need to examine her. The examiner will not take part in the conversation.
What this station tests
- Exertional chest tightness that is getting worse and now occurs at rest needs emergency hospital assessment.
- Diabetes, high cholesterol and a father with an early heart attack raise the likelihood of a cardiac cause.
- Check allergy and bleeding risk before advising aspirin, and say why you are giving it.
- Do not let a patient with possible acute coronary syndrome drive herself; call 000 and stay with her.
- Say clearly what worries you without claiming a diagnosis you cannot confirm.
