Checking a prepared tray against the original orders
PEBC OSCE practice station
Prescription review · 2 min to read · 7 min to perform
A written station. Free to try, no sign-up.
The task
You are the pharmacist checking the final tray of three prepared prescriptions before they are released. For each, the original prescription and the prepared item are described in the patient folder. Decide which items can be released and which must be stopped. For each problem you find, state what is wrong, what you would do with the item, and what you would do afterwards to prevent it from recurring. Write your answer in the response sheet. The assessor will not take part.
What this station tests
- Check the product in hand against the original prescription, not only the label.
- A correct label on the wrong strength is a classic final-check catch.
- Stop the item, re-prepare it and re-check it, rather than relabelling.
- Look for the same error elsewhere and report the near miss.
- Fix the system: separate strengths and review stock checks.
