You are the nurse on a cardiac ward. Aroha Tamihana, aged 71, was admitted with heart failure and is improving. Her son Hemi is with her. She has told a health care assistant she would like to talk about the plan. Introduce yourself, confirm her identity and discuss her care and discharge plan with her. Find out how she would like the conversation to go, who she wants involved and what matters to her, and respond respectfully. Record the plan and the support she needs on the form provided.
What this station tests
- Ask how someone would like to begin and who they want involved. Do not assume culture or preference.
- Cultural safety is defined by the person receiving care.
- Whānau-centred care means involving whānau as the patient chooses, with consent for sharing information.
- Offer support from the Māori health team and make the referral if wanted. The person decides.
- Use plain language, teach-back and a written plan, and make time for questions.
