You are the nurse on a cardiac unit in Ontario. Ernesto Ramos, aged 69, was admitted with worsening heart failure and is going home tomorrow. His wife is not able to attend. Introduce yourself and find out what he already knows. Teach him what he needs to manage safely at home. Check his understanding. Record what you taught on the form provided.
What this station tests
- Ask what the client already knows and how they live before teaching.
- Teach weight monitoring clearly: same scale, same time, write it down, call early.
- Always ask about over-the-counter and herbal products. Anti-inflammatories such as ibuprofen can worsen heart failure and harm the kidneys.
- Say what to do for each symptom: call the clinic the same day, or call 911 for severe symptoms.
- Use teach-back, involve family and give written information.
