You are the nurse at a community health centre in Ontario. Vikram Singh, aged 61, has walked in saying he feels unwell. A clinic assistant is at the front desk. Introduce yourself, confirm his identity and assess his symptoms. Take his vital signs, decide how urgent the situation is and act on it. Explain to him what you are doing. Record your assessment and the actions you take on the chart provided.
What this station tests
- Treat chest pressure with spread, sweating and nausea as a possible heart attack until proven otherwise.
- Call 911 early. In a clinic without an ECG, do not delay for tests.
- Check allergy and bleeding risk before aspirin, and give it only within a directive or on clinician advice.
- Stay with the client and repeat observations while waiting.
- Use SBAR for the paramedic handover and record the times.
- Oxygen is not routinely needed when saturation is normal, as it is here at 96%.
